Pulseaudio package developers, There is a small problem with the padevchooser package that has gone unfixed for some time. In short, the icon that padevchooser uses is no longer included in newer Debian based systems so it instead shows the "missing" icon.
I contacted CJ van den Berg, who is listed as the package maintainer, he told me that he no longer maintains any Debian packages, to contact this list and that his name should probably be removed from the package. I've read through the bug reports and it has been suggested that padevchooser either use a different icon or for simplicity include the icon in the package itself. Here is the icon it's trying to use that is no longer available: /usr/share/icons/gnome/24x24/stock/media/stock_line_in.png This was part of the 'gnome-themes-more' package. This bug affects both KDE and Gnome systems. Here are the bug reports I found: https://bugs.launchpad.net/ubuntu/+source/padevchooser/+bug/431015 https://bugs.launchpad.net/ubuntu/+source/padevchooser/+bug/632468 http://bugs.debian.org/cgi-bin/bugreport.cgi?bug=580771 This little update would fix any ugly blemish on several new Linux distros.
