Instead of adding a new changelog entry, you should finalize the UNRELEASED entry (using `dch -rD <distribution>`). Plus the 'Rebuild for trusty' entry is inappropriate, it's not a rebuild its a new revision fixing issues in the previous revision (which was a rebuild).
