Its a lot easier just to download the pre-compilled version of PHP. The extended package also contains the .dll used for GD.
PHP 4.3.1 Zip Package can be downloaded from http://www.php.net/get/php-4.3.1-Win32.zip/from/a/mirror The installer does not come with GD to my knowledge, hence why you must download the zip file.
