I would suggest breaking this project into the two logically seperate pieces.
If this is a windows DLL, make yourself a COM or DCOM object in VB or VC++, then call that COM object from inside PHP. The COM object will be able to do the tweaky stuff with networks, scanning, etc. Put all your business logic there - scan fila A and put it into folder B based on patient ID C. Alternatively, you can just make a command line application that does it in any language you wish, and call it with 'exec' from PHP.
