Chris, You've narrowed down the problem to your php configuration, I guess.
I pasted your form into 'http://146.176.13.99/heronweb/chris.html' and the response is fine on my server (ie my laptop). I am using PHP 4.1.1 on Win 2000Pro with Apache and I installed the isapi rather than the cgi version.
