I've never come across your precise problem, however it might be worth
checking file permissions on php4isapi.dll, php4ts.dll and php.ini and
making sure that your web user (IUSR_MACHINENAME) has read access to the
files - or as an initial test set read access for everyone.

Check that your php4isapi.dll is in the same directory as your php4ts.dll.

<wild guess> Maybe there's a problem with your IWAM_WIN2K password - I can't
remember but maybe you can set automatic password synchronisation for that -
maybe it's not set and you've changed the password. </wild guess>
