If you READ, under the first example posted on the URL I referenced, you would see mention of MAX_FILE_SIZE field... which I do not see in your HTML code. Additionally, if you look in the HTML posted in the example on the URL I referenced you would see the actual syntax needed for the MAX_FILE_SIZE field.
