You'd have to repeat the whole command for each option to be changed, which given the amount of typing involved seems a bit unpleasant. There are also historical differences between what is allowed by the SET var = value syntax and what is allowed in the parenthesized-option-list syntax. Introducing an inconsistency between ALTER and CREATE doesn't seem appetizing.
