Here's what I got working.
In CVSROOT/modules:
pgsql-earth -d pgsql/contrib/earthdistance earthdistance pgsql-server -d pgsql pgsql-server pgsql -a pgsql-server pgsql-earth
In postgres.cvsup:
# complete distribution, including all below pgsql earthdistance
Does that all make sense?
