The last case works, you just don't have enough parentheses.

regression=# create type mytype as (a int, b float);
CREATE TYPE
regression=# create table foo(f mytype);
CREATE TABLE
regression=# create index fooi on foo(((f).a), ((f).b));
CREATE INDEX
