[CVS ci] pmc compiler 2nd edition

I've put in a really long pending patch. The upcoming vtable changes are 
simplified a lot, as e.g. null.pmc is fully generated now.
Currently line numbers in generated .c files are wrong and disabled. 
Fixes welcome.
