Update the PR template
It turns out that the checkboxes in the template are turned into a task
list by GitHub. Not removing the extraneous lines means that a PR will
have a task list of 3 items forever, with at most one marked as done.
This change continues to encourages PR authors to think about perldelta,
and removes the tasklist inconvenience. Another side-effect is that it
becomes possible to search for one of the phrases and find PR that need
help with writing the perldelta entry.
