If you focus the host lens at infinity, it's not supposed to need any further adjustments. That said, I've had to dink with the focus point of the host lens to get it to reliably AF in some circumstances. >From your description of your problem, though, I'd suspect something electrical or electronic. You said you've cleaned the T/C's contacts ... you should do the same thing to the camera body's contacts, if you haven't already.
