I see what you mean, although I'm quite happy with the default setup: as I say, it's in practical terms exactly the same as my XP screen. The program list is, of course, based on those most
recently used.
I don't know whether you've heard of or use a module named 'Fences' from Stardock Corporation - it allows you to set up blocks of desktop icons grouped however you wish, and it makes keeping frequently-used items to hand a doddle. Avoids the use of the Start button entirely!

Yes, I've heard of it. I think I even downloaded a trial a year or so ago but found it didn't suit the way i wanted to work.

I think an operating system should be more or less invisible to a user - it should just 'be' and not get in the way of doing stuff. Like buying unnecessary camera gear on EBay..

That sounds odd, Brian - I have a Start button and menu on my W7
-64bit, and I don't recall using third-party software to set it up!
Apart from cosmetic changes, it's exactly the same as my XP machine's
Start design.

Hmmm. Well the Win 7 start menu is certainly different on mine.
Here's the default Win 7 start menu on my PC:

And here's the 'Classic Shell' version which looks and works the same way as my old XP start menu:

('Classic Shell' allows the user to switch back to the default menu if they want to. I don't!)

My problem with the default Win 7 interface is the way the programs
displayed on the left hand pane above the start button keep changing.
I want to fix the programs (and groups of programs) that I use most there, and I can't with the
default interface.

Reviews in our local serious computer magazine, 'Australian Personal
Computer', heartily endorse W8 for tablets, but suggest staying away
from it for desktops, and not highly recommended for laptops.
The biggest problem seem to be it is essentially
touchscreen-oriented, and that's an issue with most desktop users - I
certainly would not want to have to stretch out some 60-70cm. to
touch my monitor to make things happen! It is possible to switch to
the classic interface, but you have to do it on every reboot - it doesn't stay set.

Yeah, but third party developers can take care of that problem.

When MS dropped the classic start menu from Win 7 (and unlike Vista
didn't allow an option for users to drop back to the classic
interface), the absolutely wonderful and free 'Classic Shell' appeared
within a matter of months. I've been using it since I got my Win 7
machine and have never needed to look at the bizarre Win 7 menu.

Classic Shell now supports Win 8 so I assume it will enable booting
straight to the classic interface in that OS as well.
