Do them enough, and you just setup your lights how you like them and put the people in place. That way you aren't futzing around too much.
There shouldn't be too much experimentation when 'clients' are involved. I do a fair bit of self portraiture to practice my lighting setups so I'll know how to setup to get what I'm looking for. As for 18% gray, if you aren't using P-TTL, you'll want to shoot in manual (I shoot in manual always when using a flash). Once you have your lights setup, the lighting is not going to change from shot to shot, so there isn't much point in having the camera try to guess the exposure each shot.
