Sounds like driver error to me.
With poor design a contributing factor. The pedals shouldn't have been so
close together the drivers couldn't tell the difference, and the
transmission shouldn't have been able to be shifted from neutral to drive
or reverse with the throttle engaged full on.
More displacement between the brake & accelerator pedal will increase the
response time involved in moving your foot from the accelerator to the
brake - a good reason not to invcrease the distance
But yeah, it's all entirely the driver's fault.
