For the LX and MES (assuming hands the size of dinner plates):
Camera in right hand, base towards me, right thumb holding the back open.
Cassette in left hand, between little finger and heel of palm, film leader 
between index finger and thmb, enough showing to insert into MN.
Once inserted (about 1/2 inch-ish) slide left hand back, allowing cassette to 
move to front of hand and using right thumb to retain film in MN and engage 
with right-hand transport sprocket.
Drop cassette, into proper location if lucky.  Lock in place with rewind knob.
Turn camera so that cassette side is nearest me and use left thumb to move film 
onto takeup spool, using right thumb to maintain location on transport 
sporcket.  About 1/2 stroke is usually enough.  Remove right thumb and make 
another small lever movement to ensure that takeup is working.  Close back.  
Proceed.
