They are all below the limits of the 1/focal length rule, but not that
much so SR
should be able to compensate it, doesn't it?

So, have you ever experienced something similar?
Can this be due to SR not working properly?

Well, I checked my lens, and it didn't seem to be dirty. I was well under the 1/focal length rule, shooting at 1/250 and f/11 or f/13 with a monopod-mounted Sigma APO 400 Macro (f/5.6 max) lens.

I'm thinking it may be an interaction between being mounted and SR, even though it's never before happened with this exact same combination of equipment over dozens or hundreds of shots. At least a few dozen from almost exactly the same physical position.
