Focus screen/mirror position adjustment. If the focus screen or mirror or both are set at the wrong lateral position or aren't parallel to the film plane then what appears to be in focus through the finder won't be in focus on the film. The reference to back focus relates to the DOF at the film focus plane of the lens. WA lenses usually have a very limited DOF at the film plane (consider how little they move in/out when focussing) and subsequently tend to be worse affected by finder misalignment.
