If you have both installed on your system you could using Expresstalk: If you wanted to call a skype number or contact just type it into the edit box and hit dial. The driver is activated and dials the number and the input and output from skype are routed through expresstalk. When you disconnect through expresstalk the driver disconnects the skype call. The only thing is that as soon as the driver activates skype you lose focus in the expresstalk window and have to alt-tab back to it. It's really easy to set up if you're using NCH's pbx software(I think they call it axon), but they say you don't need that.
