Signing up with Pay Pal is fairly accessible.  I say, "fairly" 
because, and this is true of the graphical captchas themselves, the 
audio characters you hear will be garbled, kind of   like the 
clandestine short-wave radio stations or a twelfth-generation 
cassette copy.  <grin>  But if you listen carefully, especially for 
letterers like "B, C, t, D" etc. you will be able to sign up just fine.

I almost gave up on it at one time, but I saved the audio file and 
set Winamp to repeat until I got the letters figured out.

Once you've been finalized as a Pay Pal member you will be able to 
complete transactions with no problem with any screen reader.

A word about signup; what they will do is deposit a small amount, 
less than a dollar if I remember correctly, into your bank 
account.  Once your bank accepts the transaction you will then be 
validated with Pay Pal as legitimate and from then you'll be able to 
take advantage of all their services.
