It sounds like you're almost asking for voice recognition. Now, if the book were being done in DAISY, all of this would be available to you. But the feature you wanted regarding continuing where you left off is available in Winamp if you are a JAWS user. Otherwise, I'd recommend a portable MP3 player with resume capability.
