I'm curious you said you use bitmaps? You don't even have to make .ico file?
No. You can also specify a family of bitmaps with different colour depths if you want.
The only thing is that I have occasionally had problems with the bitmap format (Windows seems to have several internal variations). Like Ben I mostly use Paint to make mine, but it sometimes seems to get a little weird on what is actually in the bitmaps, so I also run them through the converter here to avoid any problems :
