Eu não tenho certeza, mas acho que em DirectoryIndex você deveria colocar
apenas index.pl ou customer.pl, sem o "/otrs/".  Exemplo:

DirectoryIndex index.pl
DirectoryIndex *customer.pl <http://index.pl/>*
