Please review your httpd.conf to locate the virtual or main host definition for otrs. If your modperl and apache::DBI are misconfigured, apache should not start because perlrequire script will return an error. Most probably you didn't include ~otrs/scripts/apache2-httpd-new.include.conf into your main or virtual host config.
