(ASAP in Rolling Meadows, IL). Are you available or someone you can vouch for able to do the work requirements listed below: Looking for a Senior Level SQL Server and .NET developer to create NEW database and design for a healthcare entity. Candidate must have strong knowledge with working on SQL Version 2005 to 2012. Qualified candidate must have strong skills with integration from/to other systems using SSID and SSRP. Candidate must also have the ability to create user front screens to pull from newly created database. Must have the ability to take requirements and turn to code quickly. Also have working knowledge of aggregating data from separate systems and combine to single database. This project will be 3 to 6 months in duration with possible extension for the right candidate. *List of Scope Specific Deliverables:* 1. PID (Patient Identification) lookup tool for Active Advice/DART. Create online form/tool that allows users to input Medicaid recipient number and the matching APP PID and associated demographic information is returned. Develop a front-end webpage that will interact with a SQL database to pull matching information. 2. Health Risk Assessment (HRA) from Sigma Automated Systems (SAS): SAS records HRAs for all new ACE members. Integrate HRA into APP ACE database, transform data (if needed), and extract/send to other systems. Processes need to be developed to support: · Ability to extract and sort patients by risk prioritization · Send notification to PCPs for patients who have scored high risk · Details TBD as in what notification should be sent (email, phone, etc) · Create file of high risk patients that will be used by case management team to manually enter patient into Active Advice (AA) application and assign patient to case manager. Automate process so file is updated as new ACE patients received. · Create file that lists the detailed HRA by patient number and is in format that can easily ‘copy/paste’ into AA application. Automate process so file is updated as new HRA’s received. 3. Provider Network file process Create automated SQL process to send provider network file to IHC on weekly basis. There are several tables that need to be joined to create extract. IHC returns a response file that we need to absorb and use to identify if any errors in submission file. Parts of this process already created but we need to automate and incorporate changes from a separate process, namely IHC applications 4. Health Needs Assessment file from Maximus. For each member in ACE eligibility file, a health needs assessment should be found in this file. Short-term need to develop process to automatically download and import file into SQL database. Long-term, need to determine if/where files will be sent and devise technical process. 5. Create (and automate) interface for supplemental files. Create automated process (download file, import, transform, export) for lead screening and immunization files.
