As far as I know, normally the default setup for today's VPN providers is to 
rent different servers in different countries with either provided IP addresses 
or their own ones. They then point their domain names to these IP addresses, 
whose targets are in different countries. And that's it, as the most typical 
setup.

A non-conventional setup is to have a central front-end server that accepts all 
connections, and then according to the clients' requests routes the individual 
VPN traffic to different servers in different countries connected to the 
front-end via private lines. But of course, except for special needs, VPN 
providers would not implement their systems that way.

I think the scenario as in your questions must be applying to the second kind 
of setup as I described.
