It looks like your index.txt contains non-unique subjects. Try to use the "unique_subject = no" option in your configuration file (which just ignores non-unique entries). Or try to manually mark the non-unique entries as revoked with a text editor, but you may corrupt your index completely if you don't know what you are doing here!
