If you must do this by email, any "forms" package could do. They don't necessarily protect the data even if they allow password protection. Once the form file is filled in, encrypt it and add it as an attachment to an email. The recipient would have to already know the password and have the tools to decrypt the attachment. Of course this is all a manual (or scriptable) process that would need attention from a programmer.
