The EVP_MD_CTX_init() and other functions are only part of the 0.9.7 version. The HISTORY section of the development tree (which is the one available at www.openssl.org) is incomplete in that regard. Please use the manual pages included with the version you are using, 0.9.6d in your case, to be on the safe side.
