Jim, I think it's just a bug in IE (that it doesn't prompt with the unknown CA root dialog). I gave up trying to figure that out and installed my CA certificate (.pem) into IE under Tools->Internet Options->Content-> Certificates->Trusted Root Certifcate Authorities and hit import. Now everything works fine.
