Look at major CA's request pages. I will also send you an example form in
private.

What is important:
You must use the newer xenroll.dll, not certenr3.dll as suggested in older
sources (also in SSLeay docs). xenroll.dll and certenr3.dll are Microsoft's
libraries for certificate generation and acceptance methods.

The major difference between xenroll anc certenr3 from user's point of view 
is that certenr3 did not protect the private keys with password and left them 
wide open in your keystore. Using xenroll is also much simpler than certenr3 
(where you had to fiddle with cookies etc). 

Creating the request page is easy, it's just a form with a simple script. 
What is more difficult is certificate loading page :)


BTW, as I understand it, mr. Gomez created both the keypair and certificate 
off-line, thus he had no need for certificate request page.
