This version deals with the IETF Last Call comments.   Text has already
been posted to the list for the two major areas of changes  - generalize IPv4
TTL language to include IPv6 Hop Limit, and add a new OAM section.  There
have been some minor edits to that new text, in particular the last paragraph
of the new OAM section has been tightened up somewhat.
