I have an incomplete example that you can use as a basis for your goal: https://github.com/Fusyong/luametatex-callback-playground/blob/master/vtypesetting_box.lmtx
In addition, I have made a module, https://github.com/Fusyong/vertical-typesetting that makes the whole page vertical. That is, you can take the PDF of the content that needs to be vertical and insert it into a new file. Unfortunately, it doesn't work in the latest ConTeXt version, and Hans promised to fix this problem.
