From what I see in the sample, they are very similar to /ActualText, but with /Alt instead (related to a marked section). Also for the expansion of abbreviations, /E is provided for marked sections (https://opensource.adobe.com/dc-acrobat-sdk-docs/pdfstandards/PDF32000_2008.pdf#nameddest=G13.2260327), the same way as /ActualText (but it expects whole words). It would be great to have synonyms added the /E information by default. I think this may be a requirement for text-to-speech conversions.
