You're correct, my original patch was too aggressive in that it would simply cache all stale (but otherwise cacheble) responses so long as "proxy_cache_revalidate" was "on". I was trying to cover the https://trac.nginx.org/nginx/ticket/778 ticket as well, even if it was not needed for my use case. I've prepared another patch that I'll submit in a moment.
