Having read some threads since this one was created, I see that the 0.18  mythbackend automatically detects that the sql database needs to be upgraded and takes care of that.

One caution if you follow Jarod's instruction and upgrade ivtv, particularly
if you have PVR-250's. After you upgrade ivtv, it no longer autodetects the
PVR-250 and assumes it's a PVR-150. This was reported on the ivtv list. The
workaround is to add "option ivtv cardtype=2" to modprobe.conf. I have two
PVR-250's so I had to dig around a little more to find out I needed "option
ivtv cardtype=2,2"
