What's your output device configured as?  (hint: should be ALSA:spdif)

In mythfrontend Setup->General ->Audio make sure 'audio output device' says ALSA:spdif, and make sure you select "enable AC3 to spdif passthrough" and de-select "Use internal volume controls"
