I am a new to MySQL. Would like to find out if there is any free software out in the market to design forms and report that links to MySQL. I am looking at a local database, forms and report.

DbForms enables developers to build sophisticated web-based database driven applications in very short time and with very little efforts. DbForms - applications are built in a manner conceptually similar to RAD - database building tools such as Microsoft Access (for Windows-based applications) or Sybase PowerSite (for web-based applications).
It also hooks up with other open source projects like jasperReports should you want to export your data into pdf form in an invoice or some such thing.
