Use 3d pythagorean theorem.
the distance d between point1 (x1,y1,z1) and point2 (x2,y2,z2) is...
d = sqrt((x1-x2)^2 + (y1-y2)^2 + (z1-z2)^2)
as for building an index for this? i have no idea. but that is how you
find the distance between two 3D points.
