Not really. The worst that happens is if you have mutt open on that folder and you move or remove a message from outside. And then "delete" it with mutt. Or you could read a message from mutt, then "rm/mv" it from outside.
Either way, when you next sync the folder with mutt (eg by changing folders, exiting mutt or using its "<sync>" command - usually bound to "$"). Then mutt can complain "file not found" because it expects to adjust the file, and you will need to quit mutt "uncleanly". This only results in some messages not getting marked as "read" by mutt, or some messages you marked for deletion inside mutt not getting deleted.
