I happened to try building manual.pdf using this year's texlive, and
was slightly apprehensive to see :

LaTeX Warning: There were undefined references.

On a very quick look through the resulting PDF, I don't see any
obvious problems.  But I'll attach a gzipped copy of the tex log in
case anybody cares.  Possibly this is nothing new (last time I did
this I knew nothing about TeX, now I know that much of what is its
logs can safely be ignored).

The undefined references are in lines 135 to 2159 of that log, there
are also many (repeated) warnings about \fancyhead, and some
warnings from hyperref at line 2676+.

I was going to attach the .tex file, but even after gzipping the
5.3MB only reduces to 237K which is still a bit big for the list.
