I found the correct format to be +INBOX.mutt

This is on Max OSX Mountain Lion, so I used this entry in .muttrc to
populate:

mailboxes `find ~/.mail/fastmail -maxdepth 1 -type d -name ".*"  -exec \
echo -n "+'{}' " \;`
