a little different but it is possible:
  set pager="xterm -e joe"

Editor joe will display cursor position.  Many editors provide this
function but I am not aware of a text pager which provides this function. 
You could also choose a graphical editor such as tea as:
  set pager=tea

You can put this parameter into your .muttrc or invoke it when you wish by
using the command line at the bottom of the mutt screen, ": set pager=tea"

You could even bind it to a key-combination/hot-key.

To return to normal replace with "set pager="
