I don't know if this helps, but if the appropriate features were compiled into mutt then it should be able to access the Exchange server directly by using IMAP or IMAPS (IMAP + SSL). The big difference is that this would leave your mail on the Exchange server, instead of fetching it to your laptop. In this scenario it can definitely perform simple username/password authentication with the Exchange server. If your server actually requires a client certificate then I'm not sure about that.
