I don't understand why there are so many problems? Are you using the macports install? The following macports install:
sudo port install mutt-devel +debug+gdbm+headercache+imap+pop+ssl works out of the box for me on 10.5.4. No issues at all running it in a Terminal, nor in a gnu screen session in Terminal.
