I use MH folders exclusively.
When I make a new folder I do it thus:
mhdir ~/p/mail/folder_name
that being where my mail lives. Mhdir is this script:
http://www.zip.com.au/~cs/scripts/mhdir
I'm fairly sure I found that an empty .mh_sequences file isn't enough.
It is safe to run that script against an already set up MH folder,
so you could sweep yours if you like. Read the script first, of course.

Can't help with that; I never switch folders. But it may well be a result of
not having done enough yet.
