I use display-hooks (a.k.a. message-hooks) to set 'pager' to either
"builtin" or "w3m" depending on the message.  The first hook is

    display-hook ~A 'set pager="builtin"'

The problem is that if I have just read a message that set 'pager' to
"w3m", the pager stays set to that even when I type '?' to get help or
type <Return> in the compose menu to view a message I just wrote.  Is
there a hook or some other way to set 'pager' back to "builtin" before
viewing the help page or other text that doesn't seem to trigger the
display-hook?

I am using mutt 1.2.5i with the following patches:

    patch-1.2,3.rfc822_terminate.1
    patch-1.2.5i.uen.appoct.1
    patch-1.2.bj.display-hook.2
    patch-1.2.bj.display_filter.1
