I'm not sure if this is mandatory or not, but my folder hooks have
single quotes around the whole argument, like:

folder-hook Sent 'set index_format="%3C %Z %[%b %d %H:%M] - %-22.22t
(%3l) %s"'

Try this out and see if it makes a difference.
