mutt hands off to sendmail (which queues the mail).  then when you connect
to your isp and give a /usr/sbin/sendmail -q then the mail is sent to your
isp smtp server for delivery.

If you are directly connected (dsl, cable etc), mutt sends to sendmail,
which sends the mail to the other guy's mailserver.
